By accessing or using the Primal Mountain Medical website and services, you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services.
The information provided on this website is for general informational purposes only and does not constitute medical advice. Completing a health assessment does not establish a physician-patient relationship.
A physician-patient relationship is only established after a formal consultation with one of our board-certified physicians and your acceptance of a treatment plan. All medical decisions are made by licensed physicians based on individual patient evaluation.
Our services are available to individuals who are:
▶18 years of age or older
▶Residents of states where we are licensed to practice (currently Utah, Idaho, Wyoming, Colorado, Montana, and Nevada)
▶Able to provide accurate and complete health information
Primal Mountain Medical provides telehealth services including hormone optimization, peptide therapy, and health coaching. Our services are provided by or under the supervision of licensed medical professionals.
We reserve the right to refuse service to anyone at our discretion, including if we determine that our services are not medically appropriate for a particular individual.
Payment is due at the time of service or as otherwise agreed. We do not accept insurance. All fees are clearly disclosed prior to initiating treatment.
Lab costs are separate from our monthly service fees and are typically charged directly by the laboratory. Medication costs may be separate depending on your protocol.
Your treatment program automatically renews under the terms of your original agreement until canceled. To cancel, we require 30 days notice prior to your next renewal date.
Cancellation requests must be submitted in writing to [email protected]. If notice is received less than 30 days before your next renewal date, the upcoming renewal will still process and your cancellation will take effect at the end of that subsequent term. You will continue to have access to services through the end of your current paid period. No refunds are provided for partial months or unused portions of a renewed term.
To the maximum extent permitted by law, Primal Mountain Medical shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services. Our total liability shall not exceed the amount paid by you in the three months preceding the claim.
These Terms of Service are governed by the laws of the State of Utah. Any disputes arising from these terms shall be resolved in the courts of Utah County, Utah.
